ATLANTA, GA: Wellcentive, a provider of value-based solutions recently announced its partnership with Affinia Health Network (AHN) for the expansion of its population health management and services.

"Affinia was an early adopter of Wellcentive's population health innovations, and our partnership has allowed us, together, to study, experience and innovate toward a comprehensive value-based care solution for the industry," said Tom Zajac, Wellcentive CEO. "This expanded relationship will support Affinia Health Network's strategy for longitudinal care coordination, patient and provider outreach, management of risk and financial outcomes throughout its market, and meet the needs of data aggregation and quality reporting under new payment models."

"We have been working to support clinical changes to improve care in our community while enabling our providers to meet new quality requirements," said Affinia Health Network Medical Director David Van Winkle, M.D. "This expansion will further our ability to take advantage of new payment methodologies, redesign and improve care, and financial outcomes under these new payment models. Our population health strategy will bring value to both our patients and our providers, addressing routine, intermediate and complex care needs."

Healthcare industries are nowadays plagued with a myriad of complexities around advanced analytics, implementing data aggregations and care management workflows. Wellcentive’s platform reduces integration cost, supports capabilities of multiple systems, and performs management actions, analysis and data aggregation. “Our strategy is to deliver a robust and broad platform that allows our customers to have the agility to meet the quickly evolving value-based payment regulations” says Kirk Elder, CTO, Wellcentive.

AHN’s clinically integrated network that includes Michigan region providers and Grand Rapids is expected to expand its level of platform in Wellcentive to solidify data aggregation. The company’s value-based strategies all through its primary and specialty providers’ network deliver care in hospital and practice settings and participate in growth within alternative payment models.
