OnePoint Patient Care has been breaking new ground in hospice pharmacy from day one. This includes identifying non-covered drugs at high risk for financial recoupment under Medicare Part D, tracking facility inactivity and medication reconciliation, and providing Deprescribing Options reports.

It simplifies the admission and medication ordering process so clinicians can spend less time dealing with pharmacy more time caring for patients.

With the touch of a button, clinicians on-the-go can quickly and securely place refill and new medication orders through the e-prescribing platform that integrates seamlessly with all major EMRs. They can track the delivery status of medication orders, verify orders in real time against the hospice formulary, screen for drug interactions and create a Prior Authorization for a non-formulary medication. In addition to managing a patient’s entire medication profile, OneConnectPoint® enables nurses to admit and discharge patients, and access a wide range of custom-designed reports critical to quality patient care, including Expiration of Schedule 2 Controlled Substances and Therapeutic Interchange Evaluation.
