Christus Extends Partnership with Wellcentive; Supervising Population Health Solutions

ATLANTA, GA: CHRISTUS Health, not for profit health service, extended its service agreement with Wellcentive for facilitating solutions and services for value based care and population health management (PHM). This comprehensive solution will enable clinical integration and risk management across care continuum for chronically ill patients.

Wellcentive gathers desperate data of patients report and combines it with analytics expertise to manage the clinical settings. With this partnership the service will be available to 10 hospitals across additional three CHRISTUS health markets in Texas, Arkansas and Louisiana.

The solution controls risk management of patient in primary care environment and collaborating across the continuum of care. The tie-up now expedites aligning physicians, hospitals and other practice sites in a consistent strategy for value based care. It further reforms clinical and financial outcomes by providing best in class treatment for patients.

It combines the data across variety of inpatient and ambulatory electronic health records (EHRs), CHRISTUS Health’s interoperability infrastructure and other systems.
